A hair transplant is decided long before the first graft is taken. It is decided in the consultation room, in the fifteen minutes where you either ask the right questions or accept a price and a date. Most disappointing results we see in second-opinion consultations did not come from bad luck. They came from questions that were never asked.

1. Who will actually perform my surgery?

This is the most important question on the list, and the one clinics answer most vaguely. A hair transplant has several steps: designing the hairline, extracting grafts, making the recipient sites and placing the grafts. Ask who does each one.

In many low-cost setups, a doctor meets you, draws the hairline and leaves. Technicians do the rest of an eight-hour procedure. A hair transplant is a surgical procedure, and it should be performed under a registered medical practitioner who is present and involved.

What a good answer sounds like: the doctor's name, qualification and medical council registration number, plus a clear statement of which steps the doctor performs personally. You can verify registration on the Delhi Medical Council or National Medical Commission register.

2. How many grafts do I need, and how did you measure it?

Graft count drives almost everything: the cost, the length of the sitting and the final density. It cannot be judged from a WhatsApp photo. It depends on the size of the bald area, your donor density per square centimetre, hair thickness and how the hairline suits your face.

What a good answer sounds like: a figure given after an in-person scalp examination, ideally with magnified scalp analysis, broken down by zone (hairline, mid-scalp, crown). Read more on how many grafts you may need.

Watch out for: a fixed "3000 grafts package" offered before anyone has looked at your scalp.

3. How much donor hair will be left for the future?

Your donor area at the back and sides of the head is a limited bank. Every graft taken is gone for good, and pattern hair loss usually keeps progressing after surgery. A 26-year-old who uses up his donor supply on a low, dense hairline may have nothing left to cover the crown at 40.

What a good answer sounds like: an estimate of your safe donor capacity, a plan that accounts for future loss (see our Norwood scale guide), and advice on medical treatment to protect the hair you still have, such as PRP therapy or medication prescribed by the doctor.

Watch out for: promises of "maximum grafts in one day" without any mention of your long-term hair loss.

4. Which technique will you use, and where?

FUE, DHI, Sapphire FUE: every clinic has a favourite name. The right answer is rarely one technique for the whole scalp. DHI gives precise control at the hairline and temples, while FUE is more efficient for large areas and the crown.

Watch out for: the most expensive technique recommended for your entire scalp with no explanation.

5. What exactly does the price include?

Two quotes that look ₹40,000 apart can end up costing the same once the extras are added. Ask for a written, itemised quote.

What should be included: surgeon fee, procedure room charges, anaesthesia, medicines, the post-surgery kit, the first wash, and follow-up reviews through the growth period. Watch out for: a per-graft rate that looks unusually low, with the graft count increased on procedure day.

6. Where will the surgery happen, and how is safety managed?

A hair transplant can take six to eight hours under local anaesthesia, with thousands of tiny incisions. The room matters.

What a good answer sounds like: a dedicated, sterile procedure room; single-use or properly sterilised instruments; monitoring of vitals during surgery; a doctor present throughout; and a clear plan if you feel unwell. You should be asked about your medical history, medicines and allergies, and some patients may be asked for blood tests before surgery.

Watch out for: no questions about your health, or a clinic unwilling to show you where the procedure takes place.

7. What happens after surgery, and can I see cases like mine?

Final results take 12 to 18 months. Transplanted hair usually sheds in the first few weeks and starts growing again from month three or four (see our recovery timeline). You need a clinic that stays with you through all of it.

What a good answer sounds like: scheduled follow-up reviews, a clear first-wash and aftercare routine, a way to reach the clinic if something looks wrong, and before-and-after results of patients with a similar grade of baldness, photographed in the same lighting at 12 months.

Watch out for: results shown only at "day 1", or photos that all look like different lighting and angles.

Red flags: walk away if you hear these

  • "Book today, the discount ends tonight."
  • A final graft count or price given over the phone without seeing your scalp.
  • No clear answer on who performs the surgery.
  • "100% guaranteed density" or "zero risk".
  • A very low, straight hairline recommended for a young patient with progressing hair loss.
  • No written quote, or extra charges that appear after you pay the advance.
